What changes

A full schedule, and nothing left unbilled.

Primary care runs on volume. A small denial rate across hundreds of visits a month is a lot of money, and so are the services that never make it onto a claim.

Coverage checked before every visit

Active coverage, PCP assignment and preventive benefits confirmed before the patient arrives.

Revenue you earned, captured

Wellness visits, chronic care and add-on services billed whenever your notes support them.

Fewer denials on busy days

Same-day visits, secondary coverage and plan rules handled before the claim goes out.

A report your whole team can read

Billed, paid, outstanding, and what we are doing about it, every month.

What we handle

Everything between the visit and the deposit

Your front desk keeps seeing patients. We take the eligibility, claims and follow-up work off their plate.

01

Eligibility before every visit

Active coverage, PCP assignment, copays and preventive benefits verified before the patient arrives.

02

Coding review

Visit levels checked against time or medical decision-making, with same-day preventive and problem visits billed correctly.

03

Medicare programs

Annual wellness visits, chronic care management, transitional care and add-on codes billed when your notes support them.

04

Payment posting and balances

Every ERA posted, underpayments against your fee schedule flagged, patient statements kept accurate.

05

Denials and appeals

Modifier, bundling, coordination-of-benefits and coverage denials worked and resubmitted before deadlines.

06

Credentialing

New providers enrolled with Medicare, Medicaid and commercial plans, plus CAQH and revalidations.

Where primary care revenue leaks

Four causes behind most lost revenue

Fix the cause, and the same problem stops coming back.

01 / 04

Same-day preventive and problem visits

When a patient comes in for a physical and raises a new problem, both can be billed, but only when the note separates the two and the claim says so. Without it, the problem visit is denied.

We check the note separates both visits, then bill both.

02 / 04

Coverage and secondary insurance

Patients change plans or have secondary coverage nobody updated. Claims go to the wrong payer and bounce.

We verify primary and secondary coverage before the visit.

03 / 04

Services the plan doesn’t cover

Preventive frequency limits, lab and vaccine coverage rules differ by plan. Missing them means surprise patient balances and write-offs.

We check frequency limits and coverage before the visit.

04 / 04

Revenue that never gets billed

Wellness visits, chronic care management and longitudinal care add-ons are often left off claims. They aren’t denials, but they’re still lost revenue.

We review visits for services your notes support but the claim left off.

Growing your practice?

Two more ways we take work off your plate

Growing primary care practices hit the same two walls: new providers waiting on insurance panels, and a front desk that can’t keep up with phones and insurance checks.

Credentialing

New provider? We get them in-network.

Every week a new provider waits on credentialing is a week of visits you can’t bill in-network. We handle it from start to finish.

  • CAQH, Medicare and Medicaid enrollment
  • Applications to the commercial plans your patients use
  • Follow-up with every payer every two weeks
$99 per payer, per provider
Virtual assistants

Phones ringing off the hook?

A HIPAA-trained assistant schedules visits, checks insurance and routes referral and refill requests, inside your systems.

  • Scheduling, reminders and same-day openings
  • Eligibility and PCP assignment checks
  • Referral, records and refill request routing
$10–$12 per hour
Built for a busy front desk

Dozens of visit types a day. Each one billed by its own rules.

Primary care sees a wide mix of visits in a day: preventive, sick, follow-up, procedures, vaccines and labs. Each has its own coding and coverage rules, and front desks rarely have time to check them all.

We work inside your EHR, with hands-on experience in eClinicalWorks and athenahealth, and take the eligibility, claims and follow-up work off your staff.

  • Coverage checked before every visitActive coverage, PCP assignment and preventive benefits confirmed.
  • Same-day visits billed rightPhysicals and sick visits both captured when the note supports it.
  • Nothing left off the claimWellness, chronic care and add-on services billed when documented.

When can we bill a sick visit on the same day as a physical?

When the patient raises a new or worsening problem that needs significant, separate work beyond the preventive exam. The problem visit is billed with modifier 25, and your note should clearly separate the two.

What is G2211 and should we be billing it?

G2211 is a Medicare add-on code for the ongoing, longitudinal care that primary care provides. It’s added to office visits when you’re the continuing focal point of the patient’s care. Many practices still leave it off claims.

Do you handle Medicare annual wellness visits?

Yes. We check eligibility for the initial or subsequent AWV before the visit, so you don’t bill one the patient isn’t eligible for yet.

Do you work in eClinicalWorks and athenahealth?

Yes, both. We’ll also work in other systems your practice uses.

Can you bill for an FQHC or rural health clinic?

Yes. FQHC and RHC billing has its own payment rules and encounter-based claims, and we have hands-on FQHC experience.
