This policy explains what information Care Revenue Management LLC collects through this website and how we use it. Patient information we handle for client practices is covered separately by the Business Associate Agreement we sign with each practice, not by this policy.

Our forms are not meant for patient information. Please don’t send patient names, dates of birth or claim details through this website.
