Denial management and AR recovery
Denied and unpaid claims, worked until they’re paid.
We go after the claims sitting at 60, 90 and 120+ days, fix what caused each denial, and appeal before the deadline. Many practices start here, because old claims are often the fastest money to recover.
- HIPAA BAA signed first
- No recovery, no fee on old AR
- Every call and appeal documented
Old claims aren’t always lost.
Most payers allow appeals for 60 to 180 days after a denial, and some claims marked “no response” were never received at all. If they’re still inside the window, they can still be paid.

Old denials and unpaid claims worked in deadline order, so the most at-risk money is recovered first.
The real cause, fixed
Each denial traced to eligibility, coding, authorization, documentation or payer error, then fixed at the source.
Fewer of the same denial next month
We change what caused the denial, so the same problem stops repeating.
Proof of every dollar
A monthly report of what we recovered, claim by claim.
From aging report to paid claim
A clear order of work, so the claims closest to their deadlines never slip.
Sort by deadline and value
We pull your aging report and start with the claims closest to the payer’s filing or appeal deadline, then by amount.
Find the real cause
Each denial is traced to its cause: eligibility, coding, modifier, authorization, documentation or payer error.
Correct, resubmit or appeal
We fix and resubmit, call the payer, or file a written appeal with supporting documents.
Follow up to a final answer
Every claim is followed until it’s paid or truly closed, with every call reference documented.
Six causes behind most unpaid claims
The code on the EOB tells you where to start. These cost independent practices the most.
Missing or invalid information
A missing NPI, wrong date of birth or incomplete claim field. Usually quick to correct and resubmit, if someone gets to it.
Timely filing limit passed
The claim reached the payer too late. Sometimes recoverable with proof of earlier submission.
Authorization missing or invalid
The payer has no valid authorization on file for the service.
Bundling and modifier errors
The service was paid as part of another, or the modifier didn’t match.
Not medically necessary
The payer says the service wasn’t justified by the diagnosis or the notes.
Wrong payer or secondary coverage
The claim went to the wrong plan, or in the wrong order.

Can you take over only our old AR and leave current billing in-house?
Yes. Many practices start with AR recovery only. You keep billing new claims, and we work the backlog.
How old is too old?
It depends on the payer. Claims past both the filing limit and the appeal window usually can’t be recovered. We check each one and tell you which are worth working.
How do you charge for AR recovery?
If we handle your full billing at 4% of collections, follow-up on the claims we bill is included. For old AR we didn’t bill, we charge 8 to 10% of what we actually recover, depending on the age and volume of the claims. If we don’t collect on a claim, you don’t pay for it.
Do you call payers or just resubmit online?
Both. Many denials are resolved fastest with a phone call to the payer, and we document every call reference number.
Can I look up what a denial code means?
Yes. Our free denial code lookup explains the most common codes and what to do about each one.
